Dear Middle School Parents,
In an effort to ensure a smooth and efficient opening of the 2007-2008 school-year, South Pasadena Middle School will be holding our annual Registration on August 28 and 29. You should have received your appointment time to register your student in a June mailing from the middle school. If you didn't receive your registration time call the middle school after August first (626) 441-5830.
For those of you that have registered before, you know that there are many district forms that must be read. District-wide we have used more that 45,000 pieces of paper each fall to communicate all necessary information to you!
New this year: In order to save paper we ask you to view all of the registration documents by clicking the links below and sign a "District Acknowledgment Form" to verify that you have read the documents.
The first link is for a "District Acknowledgment Form" for you to sign and confirm that you have opened and read all the registration documents. We ask that you sign and return this form during registration in August.
To be prepared for registration you should:
1. Update and bring in the "Student Demographic Update Information" form mailed to you in June.
2. Read the on line Registration Documents below.
3. Print and Sign the "District Acknowledgment Form"
4. Report to school on August 28 or August 29 for registration.
5. Bring in a document from one of each of the Residency Verification Document Categories found at the bottom of this
Letter.**

The following documents will also be given to you at registration but we have included them here for you viewing.
• Student Accident & Health Insurance Brochure (provided in your Registration Packet - not available online)
• Emergency/Information Card (provided in your Registration Packet - not available online)
There still will be some site-specific forms and information in a packet that you will need for registration. These packets will be available at registration.
If you will not be available to report to school on August 28 or August 29 please contact the middle school office (at (626) 441-5830) in August to make other arrangements to register your child. (The school office will be closed from July 1 to August 1.) Students who do not register will not be able to be placed in classes. You will receive a preliminary school schedule after you register on August 28 or 29. Please understand that this schedule is preliminary and that it may change prior to the opening of school. Final schedules will be distributed on the first day of school in period one.
We will also be sending you a packet of information in early August with additional important information about school from the PTA. Please look for this in the United States Mail.

**Residency Verification Documents
Just a reminder, be sure to bring to Registration one of the documents from EACH category listed below:
Category A
Deed of Trust
Mortgage Payment Receipt – dated within the past month
Rental/Lease Agreement, listing all names of occupants (including children), and the name, address, telephone number and signature of landlord/management company.
Property Tax Bill – dated within the past year
Closing Escrow Papers
Category B
Utility Bill (Example: Southern California Edison, The Gas Company, Water Bill) – dated within the past month
Time Warner Cable Bill – dated within the past month
Telephone/Cell Phone Bill – dated within the past month
Category C
Current Car Registration – dated within the past year
Car Insurance Statement – dated within the past 6 months to 1 year
Health Insurance Statement – dated within the past month
Bank Statement (or signed letter on bank letterhead with the South Pasadena address) – dated within the past month
Pay Stub showing current South Pasadena address – dated within the past month
Current Federal or State Tax Forms – dated within the past year
** For a newly established residence, for verification from Category B, a signed, original receipt or statement on letterhead from the utility company will satisfy the requirement, provided the name of the parent/uardian and South Pasadena address were service will be established are listed.
